Auto-extension

Payment terms:
Payment for the performance of work is made in stages:
- 30% advance payment of the cost of work, transferred by the Customer to the settlement account of the Contractor within 10 (ten) banking days from the date of invoice;
- the final calculation is made based on the volume of work performed and on the basis of the Contractor's price list after receiving the Final Act and signing the Act of Completion by both parties on the basis of an invoice for payment and an invoice within 10 banking days.
Delivery terms:
1. Periodic medical examination is carried out on the basis of the list of names of employees of ROSSKAT JSC provided by the Customer to the Contractor in accordance with the approved Schedule of periodic medical examination of no more than 60 people per day.
2. Departure of the fluorograph to the territory of the "Customer" for at least 4 - 5 days.
3. Registration, signing, transfer of the final act (Customer, Contractor, Rospotrebnadzor) and receipt of the results of the medical examination no more than 30 calendar days from the end of the medical examination (no later than 25.12.2020).

Comments: You need to upload the following documents:
1. The presence of a valid license for medical activities, including work (services) for medical examinations (preliminary, periodic), examination of professional suitability, examination of the connection of the disease with the profession, medical examination for the presence of medical contraindications to driving a vehicle.
2. The presence of confirmed positive and sufficient experience in the provision of services for conducting periodic medical examinations, examination of professional suitability, examination of the connection of the disease with the profession for at least 3 years.
3. The presence in the staff of the "Executor" of medical workers with valid certificates confirming their right to carry out medical activities.
4. Availability of disposable consumables (syringes, instruments, napkins, gynecological kits, etc.), disinfectants in an amount sufficient to conduct the examination in the prescribed volume. All materials and products used by the Contractor must have passports and quality certificates in accordance with current norms and standards.
5. The Contractor has mobile medical equipment necessary for a comprehensive medical examination.
