Terms of Reference for the Provider
periodic medical examination of employees of Volgogradservis LLC in 2020.
Service Provider Requirements.
1. Activities in the market for the provision of medical services for at least 10 years.
2. The Contractor has a license to carry out medical activities, laboratory services.
3. Conducting a medical examination on the territory of Volgogradservis LLC at the address: Volgograd, ul. 40 Years of the Komsomol 55.
4. Conducting a medical examination on the territory of Volgogradservis LLC using mobile diagnostic systems.
5. The presence of all specialists in the commission for conducting medical examinations on the territory of the Customer for conducting examinations on harmful and (or) hazardous production factors and (or) types of work, according to the Appendix.
6. Conducting a medical examination on the territory of Volgogradservis LLC in the period from July 1 to July 3, 2020.
7. Processing the results of a periodic inspection of not more than 20 calendar days.
8. Provision of the following documents to the Customer based on the results of a medical examination: conclusion for each employee, health certificates for each employee, general for all employees who underwent a periodic medical examination, the Final Act.
9. The number of employees subject to medical examination is not less than 130 and not more than 155 people.
10. The possibility of further examination of the Customer’s employees, after a medical examination, in the Krasnoarmeysky district of Volgograd.
11. Coordination of the Final Act on the results of the periodic medical examination in the territorial administration of Rospotrebnadzor in the Volgograd region - by the Contractor.
12. Deferral of payment according to the results of the inspection, no more than 63 days.

1. The request to ask all questions on the procedure using the ETP functionality (requests for clarification).
2. Before submitting envelopes, participants of this procedure MUST attach the Applicant's Questionnaire and documents, according to the list, simultaneously with submitting the proposal and a file in the form of a scanned copy of their commercial proposal signed by an authorized person and certified by the seal of the organization-participant of this procedure, to their proposals in electronic form. In case of non-fulfillment or improper fulfillment by the participant of the procedure of this requirement, the organizer of the procedure reserves the right not to consider the proposal of this participant.
3. The cost of services should be unchanged for the whole of 2020. (letter approval).

Note: this request for proposals is not a notice of a tender (public tender) or a public offer and is not regulated by Art. 447 - 449 h. 1 of the Civil Code of the Russian Federation, Art. 1057-1061 part 2 of the Civil Code of the Russian Federation. Thus, this procedure does not impose the corresponding amount of civil and legal obligations on the Organizer, and the Organizer reserves the right to refuse all the proposals received for any reason, or to terminate the selection procedure at any time, incurring no liability towards the participants.
