Comments:
LLC "Albazino Resources" invites you to take part in the competition for the provision of services for the special assessment of working conditions in accordance with the terms of reference.

Place of service provision:
1. Khabarovsk Krai, Polina Osipenko district, in the Amgun- Somnya interfluve, on the right bank of the Erroneous key in its upper reaches, the site of the Albazino Mining and Processing Plant (Coordinates of the Albazino uch. Latitude 52° 51 17.7, Longitude 137° 53 2.6);
2. Khabarovsk Territory, Amursk;
3. Khabarovsk Krai, Khabarovsk.

Terms of service provision:
1. Start date of services: 01.08.2022;
2. End date of services: 31.08.2022.

Stages of rendering services:
Stage 1 – Albazinsky GOK (7 days);
Stage 2 – Amursk (1 day);
Stage 3 – Khabarovsk (1 day).

Scope of services:
It is carried out for employees according to the attached list given in Appendix 1 to the Terms of Reference (39 workplaces).

The composition of services:
A special assessment of working conditions is carried out in order to assess the working conditions of employees for compliance with state regulatory requirements for labor protection, the formation of a list of measures to improve working conditions of employees and the establishment of the need to provide guarantees and compensation to employees.

Requirements for the Contractor:
1. Availability of valid accreditation of the testing laboratory;
2. Current registration in the register of organizations conducting a special assessment of working conditions
3. Carrying out the SOUT in accordance with:
- Federal Law No. 426-FZ of December 28, 2013 "On Special assessment of working conditions";
- By Order of the Ministry of Labor of the Russian Federation No. 33n dated 24.01.2014 "On approval of the Methodology for conducting a special assessment of working conditions, a Classifier of harmful and (or) hazardous production factors, a report form for conducting a special assessment of working conditions and instructions for completing it";
- By Order of the Ministry of Labor and the Ministry of Health dated 31.12.2020 No. 988n/1420n "On approval of the list of harmful and (or) hazardous production factors and works, during which mandatory preliminary medical examinations are carried out upon admission to work and periodic medical examinations";
- Regulatory legal acts in force at the time of rendering services, containing state regulatory requirements for labor protection and special assessment of working conditions.

Obligations of the Customer:
1. Provide the necessary source data;
2. Pay for the services within 30 banking days after signing the act of services rendered.

Accommodation, meals, delivery of the Contractor's personnel at the Customer's facilities:
1. When providing services on the territory of the Albazinsky GOK, meals and accommodation of the Contractor's staff are organized by the Customer at his own expense.
2. When providing services at facilities in Amursk and Khabarovsk, the Contractor independently provides its employees with food, accommodation, and household services (included in the cost of services).
3. Delivery of the Contractor's specialists to the Customer's facility Albazinsky GOK from the collection point in Komsomolsk-on-Amur and back, is carried out by and at the expense of the Customer;
2. Delivery of the Contractor's specialists from the Contractor's location to the collection point in Komsomolsk-on-Amur and back, as well as to the facilities in Amursk and Khabarovsk, is provided by the Contractor on its own and at its own expense. Expenses must be included by the Contractor in the cost of services.

We will be grateful to receive your commercial offer no later than 10:00 (Moscow time) 02.06.2022 with the following attachments:
- commercial offer on the official letterhead of the participant signed by an authorized person;
- offer card (Adj. No. 2 to the letter);
- counterparty questionnaire (Adj. No. 3 to the letter) and the reference list;
- scanned copies of the permits required for the provision of services;
- constituent documents: charter (in full in the latest edition), OGRN certificate, TIN certificate, founder's decision on the appointment of the head, letter of assignment of codes (Rosstat);
- consent to the processing of personal data (in accordance with the form) (Adj. No. 4 to the letter).

This request for proposal procedure is not a tender and its conduct is not regulated by Articles 447-449 Part 1 of the Civil Code of the Russian Federation;
This procedure is also not a public competition and is not regulated by Articles 1057-1061 Part 2 of the Civil Code of the Russian Federation;
Thus, this request for proposal procedure does not impose on the Customer an appropriate amount of civil obligations
