- completed contractor's application form in Word format
- Completed application 1 to TK in Excel format. Be sure to indicate per unit price, total cost of the products subject to the supply volume, in column 3 "Other expenses calculation algorithm", maximum amount of other expenses. Please, note, that no items can be added/deleted and the scope of works can not be changed.
- The KP card completed and signed by the head of the company (all fields are obligatory for filling)
- Extract from the Register of SRO members
In case of disagreement with the terms of the basic contract to the filled documents must attach a protocol of disagreement.
A commercial offer provided without filling in the required documents may not be accepted for consideration.
Contract awarding criteria: Availability of necessary documents for the conduct of activities, quality, timing and cost of work, terms of payment, availability of experience.
