• Free-form certificate signed by the head of the organization confirming that the procurement participant has a valid reliability rating assigned by the Expert RA agency not lower than A, or a similar level of rating according to the international scale;
• A copy of the License issued by the authorized body granting the right to carry out voluntary medical insurance (VHI);
• "Report on the financial results of the insurer" for 2015, 6 months of 2016;
• A copy of the Quality Management System Certificate of an insurance organization complying with the requirements of GOST ISO 9001-2011 (or GOST R ISO 9001-2008);
• Copy of the Rules of voluntary medical insurance;
• Programs (volume of VHI services) in VIP and Standard categories;
• Lists of health facilities that are included in the service programs in the categories VIP and Standard.
